Credentialed as a federal Rural Health Center, Daughters of Charity Services of Arkansas is dedicated to Christian values in our service as we provide quality, holistic care to individuals and communities in Southeast Arkansas. Daughters of Charity Services of Arkansas is a Patient Centered Medical Home, located in a Federally Designated Medical Underserved Area (MUA) with a Health Provider Shortage Area (HPSA). Services include medical, dental, counseling, social services, pharmacy assistance, Hispanic interpretation and diabetes self-management education, as well as exercise and fitness education.
Ascension DePaul Services of New Orleans provides high-quality primary and preventive care services and is leading innovation in community health care by improving patient outcomes and the patient experience.
Seton Center is a Catholic-sponsored human service agency located in Kansas City's urban core that helps people break cycles of poverty. Seton Center provides dental, social and educational programs to address current community needs, and tools to help support people in transforming their lives. The Center provides a food pantry, rent and utility assistance, a thrift store, and older adult services for neighbors in need.
Ascension DePaul Services is dedicated to establishing and maintaining community-based programs that increase accessibility to needed services in south San Antonio and Bexar County. Services include early childhood education, primary medical and dental care, social services, health education, preventive services, counseling, home visitation and other vital programs.
Centro San Vicente, a federally qualified health center and Patient Centered Medical Home (PCMH), provides community-based primary care health services in the El Paso area, with particular concern for persons who are poor and needy and special attention to those who are homeless. With a coordinated and holistic approach in caring for body, mind and spirit, we offer affordable healthcare (including dental care and pediatric services, as well as pharmacy, laboratory and radiology services), social services and health education programs in collaboration with other health, social service and educational agencies.
